1. Not legal advice

This service provides document preparation services only. We are not attorneys and are not authorized to practice law in any jurisdiction. Nothing on this website or in any document we prepare constitutes legal advice, a legal opinion, or a substitute for the advice of a licensed attorney.

Using Formly does not create an attorney-client relationship. If your situation may have legal consequences, you should consult an attorney licensed in your jurisdiction before relying on any document Formly prepares for you.

2. Service description

Formly uses artificial intelligence to help you prepare common everyday legal documents — starting with Freedom of Information Act (FOIA) requests — based on information you provide. You review, sign, and submit the documents yourself. We do not file, serve, or submit documents on your behalf.

3. Your responsibilities

By using Formly you agree that:

  • The information you provide is complete, accurate, and truthful to the best of your knowledge.
  • You will review every document carefully before signing or sending it, and you are solely responsible for what you submit.
  • You will consult a licensed attorney if your matter may benefit from legal advice.
  • You will not use Formly to prepare documents for any unlawful, fraudulent, or harassing purpose.

4. Payment terms

Formly charges a one-time fee per document, displayed at the time of purchase. Payments are processed by Stripe; your card information is handled by Stripe under its own terms and is never stored on our servers. Taxes, if any, are included or shown at checkout.

5. Refund policy

We stand behind the quality of what we deliver. If within seven (7) days of receiving your document you are unsatisfied, contact us at support@formlyus.com. At our discretion, we will either revise the document at no extra cost or issue a full refund.

Refund requests made more than seven (7) days after delivery, or after you have submitted the document to the target agency, are evaluated on a case-by-case basis.

6. Intellectual property

Once paid for, the final document we deliver to you is yours to use for its intended purpose without additional licensing fees. You retain ownership of the personal information and content you submit. Formly retains ownership of the software, templates, prompts, trademarks, and all other intellectual property used to operate the service.

7. Limitation of liability

To the fullest extent permitted by applicable law, Formly’s aggregate liability arising out of or relating to the service is limited to the amount you actually paid us for the specific document giving rise to the claim.

Formly will not be liable for indirect, incidental, special, consequential, or punitive damages, lost profits, loss of data, or loss of goodwill, whether in contract, tort, or otherwise, even if advised of the possibility of such damages.

8. Termination

You may stop using Formly at any time by closing your account. Formly may suspend or terminate access, with or without notice, if we reasonably believe you have violated these Terms, misused the service, or attempted to harm other users or our systems.

9. Governing law and disputes

These Terms are governed by the laws of the Federative Republic of Brazil, without regard to conflict-of-law rules. Any dispute arising out of or relating to these Terms or the service will be brought exclusively before the courts of the city of Rio de Janeiro, State of Rio de Janeiro, Brazil, and you consent to the personal jurisdiction of those courts.

10. Changes to these Terms

We may update these Terms from time to time. When we do, we will update the “Last updated” date at the top. Continued use of Formly after changes take effect constitutes acceptance of the revised Terms.

11. Age requirement

You must be at least 18 years old to use Formly. By using the service, you represent that you meet this requirement.
